You can print out an application from the PDF below, mail it or scan and send as an attachment in email. If for some reason you have trouble reading or printing from the PDF, email us to request an application and we will email one to you as an attachment and you can email it back to us. angelmail@captiveangels.com
PAYMENT
You can pay using 'Paypal', our safe credit card payment processor. You don't have to have a paypal account to use the service, you may use the credit card of your choice. Payment options and payment button is located just below this price list.Choose one of 3 payment amounts, then click the payment button. We will be notified when payment is made. OR you can mail a check or money order made out to Ron Gaebelnot Captive Angels or OLJ Ministries.
It would be wise to follow up with an email to us after you have made payment to verify who the payment is for. Don't forget to sign the documents, we can do nothing without the signatures.
PRICE LIST
Economy: $5 / year Limited space, one large photo or 2 small. Limited front page exposure if any. Standard graphics.
Premium: $10 / year Full web page or larger, 6-8 photos, improved graphics, front page exposure .
Premium Plus $25 / 1-year Photos, up to 15 medium size. As much space as required (maximum 3 web size pages), improved graphics, music player with your favorite tunes (if we can acquire them) email messages, hit counter on your page, permanent front page exposure, and lots more, see the current BROCHURE for more information. Examples of all of these member levels can be seen at the site.
